To limit the search to assisted living providers select “Residential Care Facilities” under “Long Term Care Facilities”. When the results page appears click the identification number of the facility you are searching for and then select “view consumer report” to learn more about the history of the community.

Violations and Resident Protection

Indiana provides a searchable list of licensed assisted living facilities online as well as information about violations, complaints, survey results and inspections. All information is up to date within 90 days and is located in one central location, making the information easy to find and navigate. Additionally, the state provides information about capacity for each facility, information about special licensing, payment types accepted and contact information. They do not provide pricing information for facilities.

Indiana performs inspections every 15 months and does have the ability to fine facilities if necessary.
