To find records and background information visit the Human Services Provider Directory. On the search form, choose the type of community you are looking for under “service code”. If you are not sure of the community type choose both “assisted living” and “personal care homes”. After the results populate, select “Inspection Summary” to view inspection reports.
Violations and Resident Protection
Pennsylvania offers a searchable, updated and accurate list of licensed assisted living communities as well as information regarding violations, inspections, survey results and complaints online. All information is updated within 90 days and is available in the same online location.
