Public Records and License Status

Ohio is highly transparent when it comes to assisted living records. To learn more about licensed communities in the state visit Ohio’s Health Facilities Division site. On this site you will find a list of licensed communities. Select “Residential Care or Assisted Living” as the provider type on the search form.

Please note that Ohio is currently developing a new website for consumers searching for assisted living.

Violations and Resident Protection

Ohio provides a searchable, current and accurate list of licensed assisted living communities online. In the same online location, consumers can search for inspections, complaints, survey results or violations. All information is updated within 90 days.

Additionally, Ohio provides information about special licensing for care, payment types accepted at each facility and contact information. They do not provide capacity information or the price of care at each facility.

Ohio conducts inspections every 15 months and is one of the few states that can not fine a facility for violations.
